Dr. Kleyman is currently enrolled as an in-network provider United Health Care, Oxford, Health Net / MHN, Blue Cross Blue Shield. Please contact your insurance carrier in advance for eligibility and benefits. For patients not participating in the health plans listed above, patience can pay privately. We can also provide you with the necessary invoices that you can submit to insurance carriers for out-of network reimbursement, or to be used for your health-care flexible spending account reimbursement.

Cash or checks accepted for payment.
Cancellation Policy
If you cannot attend scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the treatment as booked.
